2013 Outbreak of Salmonella Enteritidis at an Assisted Living Facility, Cohasset, Minnesota

In August 2013 routine surveillance interviews of two Salmonella Enteritidis cases that had the same pulsed field gel electrophoresis (PFGE) pattern, SE1B1 (Minnesota designation), conducted by the Minnesota Department of Health (MDH) revealed that both cases were residents of an assisted living facility located in Cohasset, Minnesota. MDH Environmental Health and the Minnesota Department of Agriculture (MDA) were notified and an investigation was initiated. Two cases were identified. Both had blood isolates with specimen collection dates of August 8 and August 14, respectively. Neither case reported any gastrointestinal symptoms prior to isolate collection. One case was hospitalized for 7 days. An MDH sanitarian visited the facility on August 28 and interviewed the manager. The facility housed 10 total residents. No additional illnesses were reported among residents or staff. Both cases consumed the majority of their meals at the facility. All food was prepared by staff in a central kitchen. The manager reported that the cases often consumed over-easy eggs for breakfast. Eggs were provided by a family member of the facility owner. On September 10, MDA contacted the farmer and found that eggs were washed by submerging them in water and that hens of varying ages were intermixed in the flock. both of these practices may increase the risk of Salmonella contamination in eggs. no testing for Salmonella was done and the farm stopped providing eggs to the facility. Investigators concluded that the most probable source of the outbreak was undercooked eggs sourced from a local farm although other routes of transmission could not be ruled out.
