Carrots 1993, non-O157 E. coli

In March, the Rhode Island Department of Health investigated an outbreak of illness among passengers of an airline flight. Forty seven of 74 passengers were ill. A meal had been served on the flight. Illness was most strongly associated with eating the garden salad, which consisted of shredded carrots, and iceberg, romaine, and endive lettuces. E.coli O6:NM (ETEC) was isolated from at least ten ill passengers. The salad had been served on other flights that day. Nine passengers on another flight where the salad had been served had also complained of gastrointestinal illness. An inspection of the airline food caterer's facilities and workers did not reveal any food handling/illness problems. Food samples were collected on this inspection and did not yield any ETEC. In April, an outbreak of gastroenteritis occurred among persons who ate at a buffet dinner served at a mountain lodge. One hundred twenty one people were ill. Illness was most strongly associated with eating tabouleh salad. The tabouleh was prepared from onions, carrots, zucchini, peppers, broccoli, mushrooms, green onions, tomatoes, parsley, bulgar wheat, olive oil, lemon juice, and bottled garlic. E. coli O6:NM was identified in the stool of eight ill persons that matched the strain that was found in the Rhode Island outbreak. Six additional ETEC serotypes were isolated. Carrots appeared to be the only food item in common to both outbreaks. Although it was known that the carrots originated from the same state, the precise farm source of the carrots could not be determined.
