On April 26, 2013 the Southern Nevada Health District (SNHD) learned of an outbreak of Salmonella I:4,[5],12:i:- infections among patrons of Firefly on Paradise restaurant and the adjacent affiliated restaurant Dragonfly on Paradise. Illness was reported in groups who ate at the restaurants between April 21-24, 2013. On April 26 the Health District closed the restaurants to minimize ongoing risk of illness. A total of 333 outbreak associated cases were identified. Fifty people were hospitalized. No one died. Outbreak cases had eaten at Firefly between April 21 and April 26. An inspection conducted at the two adjacent kitchens found that numerous conditions existed that could contribute to an foodborne illness outbreak. These included inadequate holding of food, inadequate cooling, improper handwashing, employee bare hand contact with ready to eat food, improper storage practices, improper cleaning practices, and improper thawing of food. On May 20, 2013 SNHD investigators announced that a cooked item, chorizo, tested positive for the outbreak strain of Salmonella I:4,[5],12:i:-. The outbreak was assigned Cluster Code 1305NVLVJPX-1.
