In October 2016 the Tippecanoe County Health Department, the Indiana State Department of Health, the CDC and the FDA investigated an outbreak of Clostridium perfringens infections among customers of Agave Azul, a restaurant located in West Lafayette, Indiana. Four people reported gastrointestinal illnesses shortly after eating at the restaurant on October 22, 2016. They had eaten dishes containing beef, chicken or pork. One of the four people was hospitalized. He died on October 26. Stool specimens collected from the case who died as well as another ill person were positive for C. perfringens. The Tippecanoe County Health Department conducted inspections at the restaurant. Investigators noted temperature abuse which could have contributed to growth of C. perfringens.
