In July and August 2016 Maricopa County Department of Public Health investigated an outbreak of Salmonella Javiana infections. There were 40 confirmed cases and 10 probable cases. Thirty three cases reported eating at a single restaurant, "Restaurant A," with exposure dates from July 16, 2016 to August 18, 2016. As many as 40 other restaurants or fast food establishments were reportedly patronized by patients. Findings of the investigation did not reveal a single source of contamination but unfried shrimp was suspected as a possible vehicle based on a case control investigation. Several pre-portioned, peppered samples of seafood (shrimp and halibut) tested positive for S. Javiana. Investigators did not find an unopened source product contaminated with the outbreak strain.

Government officials obtained unopened boxes of frozen shrimp from Restaurant A, an unopened box of halibut, and 8 different prepped/re-packaged shrimp samples. The freezer door handle tested positive for the outbreak strain Salmonella Javiana as did a sample of repackaged halibut. Five of the repackaged shrimp samples were positive for the outbreak strain and a sample from the unopened box of shrimp was positive for Salmonella Weltevreden.

FDA traced the frozen shrimp and a recall was issued of Censea frozen shrimp on September 19, 2016.
